By accessing or using the SARV Health website (sarvhealth.ca) or any of our occupational therapy and rehabilitation services, you agree to comply with and be bound by these Terms of Service and our Privacy Policy. If you do not agree to these terms, please do not use our website or services.

2. Services Provided

SARV Health provides occupational therapy and rehabilitation services in British Columbia, Canada. Our services include but are not limited to:

• Functional Capacity Evaluations (FCEs)
• Return-to-Work Planning and Job Site Visits
• Hospital Discharge Planning
• Work Hardening Programs
• Ergonomic Assessments and Functional Job Match Assessments
• Exposure Therapy

All services are subject to availability and professional assessment. SARV Health reserves the right to modify, suspend, or discontinue any service at any time without notice.

4. Eligibility

Our services are available to individuals who:

• Are 18 years of age or older, or have consent from a parent or legal guardian
• Reside in or are seeking services within British Columbia, Canada
• Have a valid referral or self-referral for occupational therapy services where applicable

5. Appointments and Cancellations

Booking

Appointments may be booked through our website, by phone, or via referral. A confirmation will be provided upon successful booking. We reserve the right to decline or reschedule appointments at our discretion.

Cancellation Policy

We kindly ask that you provide at least 24 hours notice for cancellations or rescheduling. Late cancellations or no-shows may result in a cancellation fee. Repeat no-shows may affect your ability to book future appointments.

6. Payment and Billing

Fees for services will be communicated prior to your appointment. SARV Health may work with insurance providers, employers, or government programs. It is your responsibility to verify coverage with your insurer prior to receiving services.

• Outstanding balances are due within 30 days of the invoice date
• SARV Health is not responsible for denied insurance claims
• Payments are accepted by credit card, e-transfer, or cheque

7. Professional Standards

All occupational therapists at SARV Health are registered with the College of Occupational Therapists of British Columbia (COTBC) and adhere to its professional standards and code of ethics. Our services are evidence-based and client-centered.

8. Limitation of Liability

To the fullest extent permitted by law, SARV Health shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services or website. Our total liability to you for any claim related to our services shall not exceed the amount paid for the specific service giving rise to the claim.

SARV Health makes no guarantees regarding specific treatment outcomes, recovery timelines, or return-to-work results, as these depend on individual circumstances.

9. Intellectual Property

All content on the SARV Health website, including text, graphics, logos, images, and design elements, is the property of SARV Health and is protected by applicable copyright and intellectual property laws. You may not reproduce, distribute, or create derivative works without our express written consent.

10. Website Use

You agree to use our website only for lawful purposes and in a manner that does not infringe the rights of others. You must not:

• Attempt to gain unauthorized access to any part of our website or systems
• Transmit any harmful, offensive, or disruptive content
